Permit Letter

Permit Application Requirements: 

Download Permit Application Form

Fill in the permit application form, affix a recent passport size photograph and sign personally.

• Enclose your passport with at least six months validity remaining.
• Enclose a proof of address (Driver license, Utilities bill or bank statement).
• Enclose a certified copy of birth certificate (if the applicant was born in Australia or New Zealand).
• Enclose a consent letter signed by parents (if the applicant is under age)
• Enclose certified copies of the parents passports(if the applicant is under age)
• Enclose a registered self-addressed return envelope.
